👉 Colin Flatt, an 81-year-old former footballer, died after a fall at Basildon Hospital in Essex 👉 An investigation found that he was given unsafe medication, injured while being restrained, and placed in inappropriate environments 👉 His family claimed that he was 'stripped of his dignity' during his time in care 👉 The investigation made 17 recommendations, including improving communication and reducing the use of restraints
