A physician who switches practices today starts a new patient portal from zero. A Nostr-based identity moves with the doctor, same keys, same reachable address, no new account for every EHR vendor to own.

A patient in Manila pays a US physician for a telemedicine consult over Lightning. Settlement finishes before the call ends — no correspondent bank, no multi-day wire, no currency-conversion cut for a middleman.

Healthcare doesn't reform from inside the insurer-centered system. It reforms by building a parallel one: DPC clinics, cash surgery centers, direct-pay specialists quoting real prices. The business model changes first. Policy catches up after, not before.

A patient's medical record shouldn't require a vendor's permission to move. With Nostr-based identity, the patient holds the keys and grants read access practice by practice, no portal migration, no lock-in. That's medical sovereignty in practice.

Medical sovereignty is a permissions question: who releases a patient's record, and on whose terms. Most portals answer that for the patient. Physician-owned practices can flip it: the patient holds the key, grants access per visit, revokes it after.

Physicians name it prior authorization, opaque pricing, fragmented records. Bitcoiners name it closed networks, extractive intermediaries, no exit. Different vocabularies, same diagnosis: a system built to prevent exit.

A DPC membership fee settled over Lightning clears in seconds, not net-30 through a card processor. No chargeback risk, no interchange cut into a thin margin. For a small practice running on monthly cash flow, that difference is operational, not theoretical.

Bitcoin Physicians Network keeps a map of doctors practicing outside the insurer-centered system: direct pay, DPC, cash or sats. Find one near you, or list your practice. Join us — link in bio.

AI can draft a differential and summarize a chart. It doesn't hold the license. Practices piloting Nostr-based record permissions still route every AI-assisted summary through physician sign-off before it moves between practices.

A physician building a direct-pay, Bitcoin-native practice often works without a single colleague doing the same thing nearby. BPN's networking initiative exists to connect those doctors to each other, so isolation stops being the default.

Two direct-pay clinics quoting a knee MRI post their price so a patient can compare before booking. The insurer-negotiated rate for the same scan varies by plan and network, invisible until the claim clears weeks later. Comparison shopping needs the price known before the visit.

A patient's message to their doctor usually lives inside one health system's portal, tied to a login that stops working the moment they switch practices. On Nostr, the same conversation runs on a single identity the patient carries to any clinician they choose next.

Physicians building direct-pay, Bitcoin-friendly practices usually do it in isolation, with no idea another doctor down the road is solving the same problems. BPN exists to put those physicians in touch before each one has to reinvent it alone.

A patient traveling abroad for elective surgery puts down a deposit before meeting the surgeon. Bitcoin escrow holds that deposit under agreed release conditions, controlled by no single intermediary, released once both sides confirm the care happened.

Parallel care isn't a boycott of the insurer-centered system. A DPC patient still uses the ER for a fracture or a hospital lab for a stat test. The point is a direct relationship for defined, routine care, using conventional healthcare only where it is actually needed.

Bitcoin Physicians Network runs on value-for-value: no gatekeeping, members support the work at whatever level fits and get education, networking, and advocacy in return. If that exchange makes sense to you, join us — link in bio.

A HIPAA release technically lets a patient revoke access, but in practice nobody tracks it once it's signed and filed. A Nostr-based grant expires on its own: the window closes at the time set, not when someone remembers to cancel it.

A DPC membership renews monthly on a card that can expire, decline, or trip a processor's risk flag. A Lightning invoice for the same renewal just gets paid: no card file to lose, no processor deciding if the renewal goes through.

A physician spends an afternoon re-entering a patient's history because the last EHR doesn't talk to this one. A Bitcoiner loses years of transaction history when an exchange freezes the account. Different data, same failure: nothing travels with the person it belongs to.

An AI assistant that settles a specialist's translation fee or a same-day consult booking can do it itself over Lightning, no invoice mailed and no net-30 wait. The clinician still owns the diagnosis and the plan. The agent just clears what's small around it.

Most Bitcoin-friendly practices are invisible to other physicians looking for one. The BPN Map is BPN's attempt to change that: independent doctors building direct-pay, Bitcoin-native practices, listed in one place. If your practice fits, it belongs there.

Most physicians experimenting with direct-pay, Bitcoin-native practice are the only one in their building who thinks about money this way. BPN's networking initiative exists to fix that: connect doctors already doing this so they stop working it out alone.

Finding a Bitcoin-friendly, direct-pay practice today means word of mouth or a lucky search. Nostr lets independent practices publish their services, prices, and availability directly, discoverable without an insurer directory deciding who shows up in it.

A direct-pay clinic can hand a patient the price of a visit before it happens, in dollars or sats, and settle it the same day on Lightning. No claim to file, no bill weeks later. The number is agreed before care, and the payment clears when the work is done.

Physicians adopting Bitcoin to build direct-pay care outside the insurer-centered system have a place to find each other. The Bitcoin Physicians Network connects doctors doing exactly this: education, networking, advocacy. Join us — link in bio.

A signed consent form can be timestamped so anyone can later verify it existed, unaltered, at the moment it was agreed. Bitcoin gives that receipt a proof no single party can quietly rewrite. In a dispute over what was authorized, the record settles it.

The insurer-centered system reforms slowly because everyone inside it depends on it. A parallel care economy does not wait: patients contract directly with clinicians for defined care, priced up front, on open rails. Reform comes from building alongside, not petitioning within.

In parallel care, the patient holds the keys to their own records. Grant a clinician access on Nostr, set the window, revoke it when the visit ends. The chart moves to the next doctor without waiting on a portal to release it, and the patient decides who reads it.

Independent practices already stripped the insurer out of the clinical relationship. A parallel care economy does the same to payment and coordination: patients contract directly, records stay portable, prices set before consent. It grows alongside the old system, not inside it.

A physician loses hours to prior auth; a Bitcoiner loses funds to a frozen account. Different words, same shape: a middleman standing between two parties who agreed. Direct-pay care and open payment rails remove the same obstacle from both sides.

A telemedicine consult for a patient two countries away hits the same wall every time: card networks, currency conversion, days of delay. A Lightning invoice settles in seconds, same units on both ends. The care crosses borders; the payment stops being the hard part.

An AI agent can pull an authorized record summary, but only records the patient granted it on Nostr, and only for the window they set. The permission is the patient's to give and revoke. The clinician still owns the judgment that follows.

Independent doctors and Bitcoiners learned the same lesson: a system you can't exit won't fix itself. Prior auth, opaque billing, records you don't control on one side; closed networks and captured value on the other. The shared fix is a direct relationship you can leave.

A specialist opinion should follow the patient, not the portal. On Nostr, one clinician can send a signed referral to another across different practices, no shared vendor required. The record of who said what stays verifiable. Coordination without a middleman owning it.

Bitcoin can settle a cross-border consult when banks will not, timestamp a consent form, hold escrow for a defined episode. It cannot store the chart, confirm a diagnosis is correct, or grant a license. Good for proof and payment, not for the medicine itself.

Ask a hospital what a procedure costs and you get a shrug and a bill three weeks later. Ask a direct-pay surgery center and you get a number before you consent. Same operation, same surgeon. The difference is who controls the price, and when you find out.

BPN runs on three initiatives, in order: education, networking, advocacy. Education first, because a physician has to understand why sound money and open protocols matter before the networking or the policy work does anything. That is the order decisions get made in.

Independent physicians: if you take direct payment, DPC, cash-pay, or Lightning, patients are looking for you and often cannot find you.

The BPN Map lists doctors practicing on sound-money principles.

What Bitcoin is for in healthcare: consent receipts, document integrity, high-value escrow, cross-border settlement when banks fail.

What it is not (likely) for: storing medical records, declaring a diagnosis true, or replacing licensure.

In parallel care, the patient holds the permissions. Records and consent move with them, not trapped in one system's portal. Grant access, revoke it, carry it to the next clinician. That control is the default, not an upgrade you have to ask for.
